Oakfield must comply with the Fundamental Standards as set out in the Health & Social Care Act 2008, but more than that we are committed to supporting people to become as independent as possible and to live a healthy, meaningful, and happy life.

Oakfield supports everyone to achieve their full potential and to lead a life of their choosing. All Oakfield staff work alongside the people we support, treating everyone equally with dignity, respect and supporting them to be as independent as their needs will allow.

Following an in-depth assessment and planning process when people come to live at one of Oakfield’s services, regular reviews of the support people need are carried out; we call these “Circle of support meetings”. The person supported can invite anyone to these meetings for example, family members, advocates, key workers from Oakfield and any other professionals working with them.  It is important that we listen to everyone so that people get the right support. We want to ensure that people have the best outcomes, are able to build social connections, and lead healthy, active lives. At the Circle of support meeting, plans are made of the support each person requires to achieve their goals. and ensure their wellbeing and choices are achieved.

Our staff work in a proactive way using person centred assessments, planning, evaluation, and reviews to continuously move towards a person achieving their goals and overcoming any obstacles that may arise. Using person centred thinking tools is a different way to have a conversation with the people we support, making sure each person’s voice is heard and is involved with how they want to be supported, identifying the things that need to change to achieve their personal outcomes and to constantly learn and ensure they have choice and control over their own lives.


At each Circle of support meeting care and support plans are updated in line with the review details and decisions. The person’s key worker will communicate with the rest of the support team any changes to support and care needs, goals, and aspirations of the individual. 

Monitoring and evaluations of the plans are ongoing to ensure that the right support is given in the right way and each person is making progress. By monitoring and evaluating, staff can recognise where changes to care and support plans are needed, or if someone wishes to change or try new things such as increase their independence in activities of daily living or try a new social opportunity or experience.

Oakfield has invested in a planning monitoring and evaluating computer package called “I Plan It”. This system is still in its infancy and once fully operational will enable support staff to detail evidence-based information so that progress, or change is recorded in line with the support, care, personal goals, and achievements. 

The I Plan it system allows for the information to be available to the person concerned and staff working with them. With the consent of each person the information can also be shared with families and other professionals e.g. (social workers, GP’s, local authority, etc). Social workers are able to keep abreast of the progress people are making and families can be assured that their loved one is leading a fulfilling and happy life.
